Transconjunctival blepharoplasty in lower eyelid aesthetics

I’m sharing a detailed explanation of the topic that includes my interview on CNN Türk for you.

What is transconjunctival blepharoplasty?

Transconjunctival blepharoplasty is a closed aesthetic surgical technique performed to eliminate under-eye bags and a tired appearance in the lower eyelid.
In classic lower eyelid surgeries, an incision is made through the skin; with the transconjunctival method, we enter from the inner surface of the eyelid, meaning the mucosal part called the conjunctiva. This way, no external incision or suture scar forms on the skin.

The main goal of the surgery is to reorganize the fat pads that protrude forward due to aging or genetic reasons. Instead of removing these fat tissues completely, we reposition them when necessary, which both reduces bagging and creates a more natural line in the under-eye–cheek transition.

In other words, we don’t just remove excess fat—we also preserve anatomical balance.

What is the difference from classic surgeries?

The eyelid structure is a very delicate area; we are talking about a millimetric field very close to the optic nerves.

No incision is made on the skin; the fat pads are treated by entering from the inner part of the eyelid. This leaves no visible scar from the outside and keeps the risk of complications extremely low.

For this reason, transconjunctival blepharoplasty is one of the safest and most effective methods.

Which patients are suitable candidates for this surgery?”

This surgery is an ideal option especially for people with under-eye bagging.

We prefer it in patients whose under-eye bags are prominent genetically or whose fat pads have protruded forward over time.

Because the skin structure is still elastic in these individuals, we can remove excess fat tissue by approaching only from the inside without making a skin incision. This means no scars, and the under-eye area gains a flatter, smoother, and more rested appearance.

In addition, this method is ideal for patients with a busy work schedule or those who come from abroad and want to recover in a short time, because the healing process is very fast and comfortable.”

Can you tell us about your fractional laser application after surgery?

In our own protocol, we apply fractional laser to the skin surface immediately after the surgery. This laser increases collagen production in the skin, helping the under-eye skin tighten and firm up. In other words, the bags are removed and the skin tissue is renewed—resulting in a smoother and more vibrant appearance.

How does your patients’ recovery process progress, and how long-lasting are the results ?

Because there are no incisions or stitches with this technique, recovery is quite fast and comfortable.

First 24 hours: Mild swelling and bruising may occur

Around day 3: Most of our patients return to daily life, and patients coming from abroad can safely return to their flights on the 3rd day.

As for longevity;

Thanks to the elimination of fat bags and tissue renewal supported by laser, results are long-lasting for many years; in most patients, we see satisfaction for 8–10 years and beyond. Of course, biological aging continues; however, a pronounced return of bagging is not expected early in most people. A healthy lifestyle, regular sleep, and sun protection increase longevity. If needed, in later years, laser maintenance treatments or supportive procedures that improve skin quality can make the results even more long-lasting.”
